Q. How do I know if I am eligible for FMLA?

A. You may eligible for FMLA if:

  • You have been employed for at least 12 months; and
  • Worked at least 1,250 hours during the 12-months immediately before the first day of leave.
  • Experiencing a qualifying family or medical reason.

Q. How do I request FMLA?

A. Contact your supervisor if you are interested in FMLA. Your supervisor will contact HR for more information regarding your eligibility.

Q. I was just injured at work, what should I do?

A. You must notify your supervisor immediately after the incident.  They will provide you with an Accident/Incident Report to fill out and sign along with further instructions provided by HR.

A. You are qualified for RSI health insurance.  HR has provided information to your supervisor for further details on enrollment, premiums, and dates of eligible enrollment.

Q. Who do I contact when I have a complaint with one of my co-workers or supervisors?

A. If the complaint is regarding a co-worker, then contact your supervisor for resolution.  If you have a complaint concerning your supervisor, please contact your Operations Manager or Human Resources.
